I’ve just found a copy on the network drive at work that the local NHS sent to us! BTW anything from 5-7 and there is something wrong with the patient…

it says 5-7 are actually evidence of diarrhoea and in somewhere like a residential home that has to be noted for infection control purposes (which is fair enough given the circumstances).

the bottom half contains a grid for patient info and frequency of bowel movements and the top bit is exactly the same as what you posted up there. it was as its name suggests developed at the teaching hospitals associated with the University of Bristol but is famous worldwide…
